Moderna: (2 dose) for ages 18+
Pfizer: (2 dose) for ages 12+  
•    Ages 12 -14 must be accompanied by a guardian/parent for Pfizer
•    Ages 15+ do not need to be accompanied by a guardian/parent for Pfizer
Johnson & Johnson: (1 dose) for ages 18+
                                                                    
Please wear a short sleeve shirt for immunization.
